Office Extension, Not a Competitor
We are explicit about this: Conci is not a primary care practice. Your patients stay your patients. Every visit closes the loop with a note back to your office so the relationship and the chart stay intact.
You Stay In the Loop
You order the test, the visit, or the procedure. We execute. The note, the result, and any follow-up recommendations come back to you within the SLA we agree on.
Physician-Led, Coordinated Care
Visits are delivered by board-certified clinicians under physician leadership. Your patient gets a higher-touch experience without losing the continuity you have spent years building.

Morning Fasting Labs in the Patient's Kitchen
Your patient needs a fasting lipid panel but cannot afford to miss the 6 AM Metra into the city. You order the labs in your EMR. We dispatch a phlebotomist to their home in time to draw, label, and transport the specimens to your preferred lab. Results come back to your office on the same workflow you already use. The patient never reschedules a meeting, and you do not lose another week chasing a no-show lab.
In-Office Subspecialty Days
Want to offer orthopedic evaluations to your panel without referring out? We bring a Conci clinician with the right training, the right kit, and the right paperwork to your office on a recurring schedule — say, Wednesday mornings. You introduce the patient, we evaluate and document, you keep the relationship. Same model works for dermatology consults, joint injections, and other in-suite subspecialty visits where it makes sense.
Sutures, Procedures, and IVF Support — Brought to Your Office
Some procedures are easier when we come to you instead of asking the patient to travel. Lacerations, suture removals, abscess care, skin biopsies, and IVF-adjacent injection support can be delivered in your suite by a Conci clinician on a scheduled or as-needed basis. You decide what to keep in-house and what to hand off — we just make it possible to keep more of it in-house.
Overflow & Same-Day Surge Capacity
It is Monday morning, the schedule is full, and three patients call sick. Instead of telling them to go to urgent care across town, you offer them a same-day Conci visit at home. The patient feels cared for, the encounter and note come back to your office, and you keep them out of an ED waiting room. Often same-day; timing depends on demand and location.

You Order, We Show Up
Orders come in through a simple secure channel — message, portal, or your existing referral workflow. We confirm dispatch, send the right clinician, and execute the visit on the agreed timeline.
Notes & Results Come Back
Encounter notes, results, and recommendations come back to your office in the format you prefer. The patient stays yours; the chart stays yours; you stay in the loop.
The Boundaries We Will Always Respect
We are clear about what we do and what we do not, because the partnership only works if you trust those lines.
We will
Execute the visits, draws, evaluations, and procedures you order under the SLA we build together. Send notes back. Loop you in on anything that needs your attention. Refer your patient back to you for follow-up — never pull them into a competing primary care relationship.
We will not
Position ourselves as your patient's primary care provider. Make outcome promises we cannot keep. Hold a patient longer than the visit requires. Charge insurance directly — we are out-of-network, cash-pay, with a superbill provided so the patient can submit for any reimbursement they are eligible for.
